As a very concerned friend and one that has the background to realize that what my friend is undergoing is bordering on severe depression, I   know that she should really seek professional help. Although therapy is expensive, there are certain measures that could be used as an alternative to therapy, but I would be more comfortable if she will go to counseling.This is why I researched a number of internet resources to really identify the best sources for reputable and accredited psychologists, as much as possible I would want her to not be under medication because in counseling she would have a better chance of developing the skills she would need for healthy adjustment to life’s challenges.In psychotherapy, she would be given anti-depression drugs and sometimes it is not as helpful as it is meant to be, and I fear that she may only become dependent on the drugs instead of really learning and realizing her problems and issues.I found the Psych Central website very informativ e and helpful. It has a list of help centers where the fees are not that expensive and some of the sessions can be done online, on the other hand, it also has a list of accredited psychologists and psychiatrists and their contact and office numbers as well as descriptions of their fields of expertise and work experience.I simply went online to find the resources, aside from Psych Central, I also found that San Jose, CA has a list of psychiatrists and psychologists on the yellow pages, but unless I have someone who can refer them, it is not much help.From the Psych Central resource list, I found two probable psychologists that are within the San Jose area, they both offer the first session free of charge and present a very comprehensive introduction to their practice. I feel comfortable with the approaches that the psychologists have on treatment and they have outstanding credentials.One offers a sliding fee and charges $110 to $119 per session and can be bargained if financially inc apable, likewise there is no need to pay for the sessions in advance which would suit my friend’s needs. The other site however does not indicate the schedule of fees.They both work with individuals in counseling and specialize in depression and adjustment problems. The first psychologist however accepts payment sessions through health insurance and she says that the client just have to contact the insurance provider before arranging the first session. This could be advantageous to my friend if she decides to use her insurance plan but I doubt that she would, but it is good to have options.The second therapist however requires that a personal call or an email be sent to her to settle the business side of the counseling sessions, which is basically positive in the sense that my friend would get to communicate with the therapist before she commits to visiting her and her practice is also within the San Jose area and can be checked out anytime if the need arises.I think that my friend would really benefit with the first counselor, she would be given the counseling that she very much needs within her own budget constraints, as well as be able to stay in her apartment and job for the psychologists are within her residence and job. Do video games contribute to teen violence? Essay

Two teens in black leather coats waited with their vehicles near the west entrance of Columbine High School. The teens, Eric Harris and Dylan Klebold , headed toward the school. Armed with four guns, several knives and dozens of home-made explosives, they launched their assault, injuring several students and killing two before entering the building. The school shooting that took place on April 20, 1999, was evidently a very devastating and emotional day, but what many fail to comprehend is what led these two students to such drastic measures. After various studies, and concrete evidence, a rational explanation may be that these two killers were fanatical players of violent video games. Unfortunately, the destructive impact violent media has on today’s youth doesn’t end there. Violent video games have been known to cause aggression in players, increased reports of bullying, violence and ruins the virtuous morals instilled in today’s generation. Studies support that there is a strong link between violent video games and aggressive conduct. Many children that take part in playing violent games often report feelings of aggression and violence. According to the Michigan Youth Violence Prevention Center, â€Å"It is intuitive that young kids who play violent video games, compared to young kids who do not play violent video games, will be more aggressive in certain real-life situations. † In other words, children who play violent video games tend to see the world from a hostile perspective, triggering them to be verbally as well as physically aggressive. This is obvious considering the fact that violent video games portray violent behaviors and suggestive material. Procon states, â€Å"A 2009 study found that it takes up to four minutes for the level of aggressive thoughts and feelings in children to return to normal after playing violent video games†. These four minutes of aggression is enough to compose violent thoughts in children. Exposing our adolescents or even our general population to these sorts of behaviors can lead to serious and possibly fatal consequences, for instance bullying and school shootings. Violent media can be very influential and unhealthy exposure may result in disastrous consequences, such as bullying. Though it may seem extreme, studies reveal that after just 10 minutes of violent video games the mind becomes desensitized to the fact that you’re killing someone, and according to psychologist Michele Borba, it can also result in less empathy towards others . Adolescents that play violent video games tend to associate pleasure with the ability to cause pain in others(â€Å"Do Violent Video Games Contribute to Youth Violence†). Players can sometimes confuse fantasy with reality and at the very least, their brain finds happiness in the suffering of others, which could possibly lead to bullying. The 2008 study Grand Theft Childhood reported that 60% of middle school boys who played at least one Mature-rated game hit or beat up someone, compared to 39% of boys that did not play Mature-rated games (â€Å"Do Violent Video Games Contribute to Youth Violence†). Young people that bully others often times are also involved in playing violent video games, and the violence portrayed on screen, lead players to violence in real life. Not only do violent games contribute to bullying, scientists also suspect that violent games may play a role in school shootings and other massacres. One of the most extreme, and disturbing effects of violent media is evident in school shootings such as the Newton Elementary Shooting, killing 26 first graders and the Columbine Massacre, killing 13 students. The Hartford Courant has reported that, â€Å"Adam Lanza, Newton Elementary school shooter, spent hours playing graphically violent video games in his basement before the shooting that killed 26 children and faculty at the school and Lanza’s mother at her home. † Obviously, Lanza was influenced by the violence in the video games which then caused him to commit these horrific murders of young, innocent children. According to the Carey Benedict, a journalist for The New York Times, families of the victims of the Columbine shooting sued Sony, SEGA and Capcom for the violent video games the gunmen, Eric Harris and Dylan Klebold, played. Klebold and Harris were avid players of violent games, The New York Times states, â€Å"They [Klebold and Harris] relied on(†¦) games to express their rage and(†¦) cutting them off in 1998 sent them into crisis. † When the teens lost their priviledges, they went on a violent rampage to express their rage and committed a mass murder, The gruesome images portrayed in games can possibly lead to extreme mental issues and destroy the morals we strive to instill in our youth. Our adolescents today will create an impact on tomorrow but our hopes of a bright future may be marred due to the violent nature of video games. Video games cause a decrease in sensitivity and compassion in adolescents, making them less caring and unsympathetic. The American Psychological Association states, â€Å"playing violent games is also related to children being less willing to be caring and helpful towards their peers. † Video games decrease compassion in kids, and could possibly lead to an insensitive, hostile generation. In addition to reducing sympathetic emotions, video games also allow children to witness foul language, teaching them vulgar words which could influence them to use them in real life (â€Å"The Positive and Negative Effects of Video Games†). Another effect, violent media has on today’s generation are gender inequality. In a majority of video games, women are often portrayed as weaker characters that are helpless or sexually provocative. Procon has stated that, â€Å"A 1998 study found that 21% of games sampled involved violence against women. † Victoria Beck, a professor at University of Wisconsin , states that, Video games showing sexual objectification of women and violence against women resulted in statistically significant increased rape rape-supportive attitudes for male study participants. † This proves that exposure to sexual violence in video games is associated to increases in violence directed at women and fake attitudes about rape such as, women incite men to rape or that women secretly desire rape. This sort of an outlook on women could greatly harm our adolescents and laws of today’s and tomorrow’s society, possibly causing rape and therefore, leading to all sorts of violence. Insensitivity and rape, these are only a few of the critical effects of violent media, but what exactly is the cause of these disasters? It is apparent that playing violent video games has many destructive effects, but the serious psychological effects are often overlooked. Douglas A. Gentile, a psychologist in Handbook of Children, Culture and Violence, states, â€Å"In a series of studies with children adolescents found that video game violence exposure was related to a wide array of aggression. † Video games seemingly cause aggression in people, and extended exposure could put our future in jeopardy. People who played video games obsessively could also become socially withdrawn, like the shooters in the Columbine Massacre. Constant exposure to digital violence could fuel their minds and give them the idea that it’s okay to commit acts of violence. The opposition may claim that, not everyone is affected by the violence presented in some video games, but violent media affects everyone, at least, to some extent. For example, playing video games could be compared to smoking cigarettes. A single cigarette won’t cause lung cancer, but smoking over weeks or months or years greatly increases the risk. In the same way, repeated exposure to violent video games may have a cumulative effect on aggression. Though, only one third of smokers get cancer that does not necessarily mean that smoking is acceptable for the other two thirds of smokers. Similarly, video games may only have an extreme effect on those with mental health issues, but this does not mean that the repeated acts of violence portrayed in these games is suitable for others to observe, in fact, the repeated exposure could have a damaging effect over time.

He has all the Characteristics of a Byronic Hero. This poem constantly takes me back to the 1800s, because at that time women had no rights at all. Julia married Alfonso only because she had to. Alfonso never showed his jealousy because nobody likes a jealous person.Alfonso cared about what people thought and he was cleaver and careful. He never wanted people to know about his illegal affairs. I personally think Donna Inez is a psychopath and a Sociopath both. I mean why else would she use her son to destroy the relationship of her lover and his wife. Julia is not as complicated as Inez. Julia does not realize what Inez had in her mind. I think she thought Inez actually wanted to be her friend. Being a simple girl Julia probably didn’t know about Alfonso and Inez’s affair. When Juan was a child Julia showed affection for him but that was just normal and innocent.When Juan hit puberty something changed between both of them. They felt attracted to each other. As a grown woman Julia realized why she was feeling this way. But Juan is completely different. His mother never let him mix and mingle with people of his own age. At first he didn’t even realize what was happening to him. This poem follows a third person point of view. In these stanza’s Byron talks about lot of important issues but the tone is always humorous and satiric. Don Juan is a lot like Byron himself. The difference is Byron was highly educated and Juan had no interest in that line.

The poem tells the story of a man in a mental institution, who exceeds the expectation of both the nurses and his fellow patients when he regains the ability to talk. In the first stanza of the poem, Gillian Clarke describes the country house in what seems to be an idyllic setting, The sun treads the path among cedars and enormous oaks, it might be a country house, guests strolling. However despite the seemingly pleasant tone, implied by the use of her making it seem informal, through the relaxed wondering of what the House may have been, might be a country house, guests strolling, suggesting normality her use of the word might alerts the reader that this idyllic setting may be an illusion and not what it first seems. The illusion of normality is swiftly extracted by the opening line of the second stanza, I am reading poetry to the insane. This line ends with a certain finality, that is so abrupt that it disturbs the so far, flowing effect to the poem, also implying informality and normality, to the effects that it shocks the reader, not only in the disrupted rhythm of the poem but also in the disturbingly blunt reality of what she is saying. Furthermore this is shocking because it is not commonplace for people to be reading poetry to the insane. Gillian Clarke does this numerous times during the poem in order to stop the reader, so that the poem does not ramble, and make it more interesting, A beautiful chestnut-haired boy listens entirely absorbed. A schizophrenic. This use of contrast between the descriptive lines of the opening stanza and the flat, and remarkably blunt, tones of this line introduce the reader to the contrast between the setting and the guests. As we can observe from the line following, I am reading poetry to the insane, which instantly restores the mood of the poem to informal with the humor, of the old woman who is constantly offering the narrator coal, when it is March and she would have no means of getting coal. By the use of having the woman saying humorous things, Gillian Clarke is also contrasting her with the other patients at the home, as she is the only one who is talking. The poem also uses poetic devises such as personification, An afternoon yellow and open mouthed. It uses metaphors, In a cage of first march sun, and similes such as, Outside the daffodils are still as wax, to make the poem more interesting to read, and also without these techniques the imagery of the poem, that the people in the home are daffodils would not be portrayed and the final message of the poem would not be presented to the reader. Thus the poem would be pointless. Gillian Clarke also uses enjambment in the poem, which disturbs the flow of the poem, and I think is also relating to the disturbed personalities of the patients in the home. Enjambment is effectively used in the third to fourth stanzas, as this is a rambling scene of the immense man struggling to get to his chair, and so Clarke reflects this in the appropriate use of enjambment between the stanzas. The first March sun is described as a cage in the third verse as it is saying that for these people who have no fre edom, even their enjoyment of the sun is trapping them, and they have no choice but to be out absorbed in it. These poetic devices are also used to build up the character of the man in the poem. In the third stanza where the man is introduced, he is described as a big, mild man, and a laborer, who is being tenderly led. This use of contrasting language informs the reader that even though the man is large in size he has to be tenderly led like a child, continuing to imply to the reader that there is something wrong with him. This is confirmed in the forth verse where he is described as rocking, a common action by insane people as it comforts them. His description is also emphasized by the repetition of the words big, mild and dumb. These words make him seem even more immense, which is odd when they are combined with mild. When the huge laboring man speaks he is in beautiful surroundings reciting a poem about daffodils, which is not only being ironic but is breaking the stereotype of laborers being very masculine and rather insensitive, whereas here he is being portrayed as almost feminine and extremely sensitive. What strikes me most prominently when reading the poem is the amount of imagery used to bring emphasis to the overall meaning of Clarkes poem and to make it more interesting. She has chosen the Daffodils, by W. Wordsworth, as the music that the mute man chooses to speak after forty years of silence. I believe that she has also chosen to describe the man who cannot speak as mute as it is also a musical term, and so therefore is not just emphasizing that there is no speech in his life, but also that there is no music, relating with happiness and merriment, and therefore is saying that without speech there is no joy in the laborers life. However when the rhythm of the poetry he is read awakens him, it appears to turn apparent life into reality. The poem is a possible way to show his waking from a world of misery to a reality of natures beauty and rhythm, Since the dumbness of misery fell he has remembered there was a music of speech and that once he had something to say. This is also playing on the word dumbness, as dumbness aside from meaning stupidity also is relating to the man who cannot speak as a person who is mute can also be described as, dumb. Informing us that the poem is saying that music is good, as when he cannot speak, and there is no music, it is described as, misery and when this, falls, and he has remembered that there is something to say a thrush sings, representing happiness, and the once, wax still daffodils are flame, representing excitement, life and activity. Therefore Gillian Clarke is saying that the man was not properly alive until the music in his life was restored. He is reciting poetry because what he has heard from the nurses in the institution has restored his memory and he has remembered a poem that he had learnt as a child at school. We know this because Clarke informs us in verse thirty-one, Forty years ago, in a Valleys school, the class recited poetry by rote. I believe that Gillian Clarkes poem is also relating to music through this imagery. Since the dumbness of misery fell he has remembered there was a music of speech and that once he had something to say. When he speaks there is an immediate exchange of characteristics, the once lifeless patients are alert and the nurses are frozen as the patients once were. I also notice that in Clarkes poem the daffodils seem to represent the people at the home. At the start of the poem the daffodils are open mouthed showing the way that the patients dont react to the poetry, as this is the face that people use when they are bored and not listening. Their open mouths show how unreceptive the patients are, once again removing any sense of normality as Gillian Clarke alienates them from ordinary sane people. When the miracle of the man speaking occurs the flowers are silent and still, showing that far from the boredom and lack of interest displayed before, everyone is amazed. We can deduct that the daffodils are not merely flowers, by the use of lines such as, their syllables unspoken, as obviously, flowers can speak no syllables. I think that the use of the candle related words throughout the poem are deliberate, when the flowers are as still as, wax, the man is not speaking and when he does they are a flame, which is representing that the mans hope, which has always been there, the wax, but when he speaks it is a flame. It is alive. I think that the, first bird of the year in the breaking darkness is symbolic as the first bird of the year, is representing the first speech in the mans life for many years, and the darkness which is now breaking is the bad times of when he could not speak. His first words in many years are described as a bird, as this also relates to the theme of music. Gillian Clarke effectively alienates the patients in the home and portrays the fact that although they are physically there, they mentally are not by the constant use of word absent, I read to their presences, their absences. St Davids Day by Gillian Clarke illustrates the theme of identity through the use of including people in a mental institution, as they have no identity. Yet after forty years a mans identity breaks through. I believe that this poem is trying to communicate the fact that everyone has an identity; no matter how masked it is from the rest of the world, and by the use of describing them with flowers that cannot speak (which have inner beauty, she is saying that everyone has some kind of identity and beauty. Emotion plays an important part in the poem humor, misery and shock are shown to us and this makes the poems more realistic. Ultimately I feel the reason this is a very effective poem is that the use of daffodils and Wordsworth is subtle but carries a significant meaning that is backed up by the tone of the rest of the poem. I find that the poem is truly touching and although the main message of the poem is quite discrete, the way that it is portrayed makes it seem that you, the reader are really experiencing the miracle.

Origins of Visual Expression in Art In this dissertation, I will research the origins of visual expression, firstly asking why was it made, and who was it made for? I will then be looking for the earliest examples of where visual expression was found and then be moving onto how western art was developed and controlled though the early centuries in Europe. Then focusing on how one artist (Marcel Duchamp) chanced how institutions and art galleries could value art and lead the way for the future artists. Finally, this will bring me onto look at the emergence of the YBAS (Young British Artists) and the dynamics of their surroundings which would evidently lead to their success in the international art world, helped by the modern systems of mass media, written medias and public opinion. In this, I will investigate the relationship between visual and modern day language mediums thought discourse, with an added envious on my own personal experience though the viewing of the art works in the exhibition at Liverpool Tate Bad art for bad people? by Dino and Jake Chapman (13th December 2006 â€Å" 4th March 2007), also including another artist who was the forerunner of the YBA movement and who would later become the YBA pin up, Damien Hurst. I will look how Damien Hurst used the systems of the media, being, news papers, Magazine columns, visual media and mass audiences, to make his own unique stamp in the art world. I will look how the YBAS became into the international limelight during the late 1980s and 1990s, helped with the guidance and backing of the advertising mogul Charles Saatchi, and his effect on the mass audience of the British public and aboard, making the YBA movement a success in the worldwide art scene. (1917) and how it can be singled out as the bench mark for future contemporary art practice of today, firstly looking upon the history of Marcel Duchamps life up to the point of his work The fountains? creation and existence in 1917. (3A) Marcel Duchamps Fountain 1917? www.google.com/fountain.gif/cwru.edu Marcel Duchamp was born into world, in 1913, the French writer Charles Peguy Remarked, The world has changed less since the time of Jesus Christ than it has in the last thirty years.? (18) He was speaking of all the conditions of western capitalist society: its ideas, its sense of history, its beliefs, modes of production, and its art. Born on the 28th July 1887 in Blainville, near Rouen in France, He was the brother of Raymond Duchamp-Villon, the sculptor, and of Suzanne Duchamp, the poetess and also half- brother of Jacques Villon. He began to paint in 1908 and at the age of 22 Duchamp was a member of The golden circle? a painters circle, which included Metzinger, Leger, and Picabia, He was painting in the style of Cubism and futurism, which is shown his work nude descending a staircase,? painted in 1912. (19) In 1913 Duchamp exhibited this work in the New York Armoury show, in which it was the much more ridiculed work at the show. In 1912 when he painted Nude descending a staircase? Duchamp said, that painting is washed up?. (20) In abandoning painting, he said, I want something where the eye counts for nothing.? At this point the Duchampian revolution consists of the notion of the ready-made?. (20) This term describes common objects with or without modification that were relocated in museums and galleries. The term objet trouve? first was first recorded in a letter to his sister Susanne Duchamp in 1913. (21) The earliest readymade of Duchamps was the Bicycle wheel of 1913. This consisted of a bicycle wheel fixed onto a wooden stool. These readymades ojects were mass produced objects with common uses such as snow shovels and bottle racks which Duchamp would then sign. He would give the objects names that were totally irrelevant for their practical use. In 1915, Duchamp went to the USA for the first time. (22) The USA now better developed than Europe in technology, communications and now had the tallest building in the world. Soon, Duchamp settled and became the centre of a group of painters round the Stieglitz? gallery in which the group adopted the anti-art? attitude as with Zurich Dadaism. (23) In 1917, Duchamp sent his mass- produced urinal, (readymade) called fountain? to New York, (where the first show for the society of independent artist was held. Simply called independent show?, (24) signed with the name R.Mutt?, it was nothing but a common urinal. The work that was signed with a false name and exhibited on its back became centre to an unrealistic approach and was pulled out of the exhibition. The theory behind the readymade explained in an article, anonymous but believed to be by Duchamp himself in defence to his alter-ego, Mr Mutt, In the May 1917 issue of the avent- garde magazine The Blind Man?, run by Duchamp and his two friends, printed this text. Whether Mr Mutt with his own hands made the fountain or not has no importance. He chose it. He took an ordinary article of life, and placed it so that its useful significance disappeared under the new title and point of view-created a new thought for that object. There are three important points here: First that the choice of the object is itself creative act. Secondly, that by cancelling the useful? function of an object it becomes art. Thirdly, that the presentation and addition of the object have given it a new thought?, a new meaning? (25). Duchamps readymade also asserted the principle that, art is defined by the artist. The idea of art is made in the artists mind as a concept for their personal perspective of how their world is interpreted. This is true for every person, as everyone can think of ways an object means more than its physical form. This is also true of objects that have sentimental value. In any place the owner has the object in owe of what it represents regard of its space. As objects of no meaning are only then represented by the space the object inhabits. Marcel Duchamp gave the world a diverse outlook on the way art and the object could be perceived and portrayed in society, therefore leaving the door of the art world and its individuals open to create and develop concepts and ideas of art, which left the old institutions and practices to be questioned and revaluated. From abstract expression to the YBAS, Marcel Duchamps Fountain can be seen as a turning point in the ideas of where art can take the artists and the viewer. This can be seen none more so than the young graduate group of British artists of the late 1980s to take the international art world by storm, they were later to be known as the YBAS. Duchamps single act of artistic expression changed the way we could view art but to explain this he had to enforce it with words. Discourse and written language had more importance now rather than just the documentation of visual art and its history. Written language now became a factor in which works of art were viewed, and with the evolution of the tabloids and the mass media would become intertwined feeding off each other for publicity, good or bad.

Environmental And Family Factors Social Work Essay It is important to note that pseudonyms have been used throughout the case study in order to protect confidentiality, in adherence with the NMC (2008) Code: Standards of Conduct, Performance and Ethics for Nurses and Midwives. Health visiting practice involves the planning of activities aimed at improving the physical, mental, emotional and social health and wellbeing of the population, preventing disease and reducing inequalities in health (DoH, 2006). Children who come from families with multiple risk factors and are even more likely to be disadvantaged in terms of poor health and social outcomes such as developmental delay, behaviour problems, safeguarding concerns, mental illness, substance misuse, teenage parenting, low educational attainment and offending behaviour (WAG, 2012). The role of the Health Visitor is to identify children and families that are at high risk and or have low protective factors and to ensure that they have a personalised service to meet their needs. The services required may range from one off interventions to long term intensive support. The Health Visitor will ensure that adequate supportive resources are allocated to the family and will engage and work with key partners/agen cies to meet the wider health needs of the children and families (WAG, 2012). This case study will explore the needs of a family within the student health visitors practice case load. Lisa, a 24 year old mother of three, was living in a third floor flat without a working lift. Her partner Chris was working fulltime so she was left to alone look after a baby and three year old twins. This meant Lisa was effectively trapped in the flat as she felt unable to safely move all the children downstairs to get out. This came to the attention of the health visitor who was concerned that Lisa was 7 weeks postnatal and had not been attending well baby clinic with the baby as planned. Consequently a follow up home visit was arranged to compete the babys 8-week check and it was an opportunity for Lisa to complete the Edinburgh Postnatal Scale (EPDS) as per local trust policy. During the follow up visit Lisa disclosed to the student and Practice Teacher that she had been feeling low and was finding it difficult to get motivated. Lisa was aware that not leaving the flat durin g the week when Chris was at work was having a negative effect on her own wellbeing and her childrens development. She stated that her relationship with Chris had become increasingly strained; he was working long hours at a local factory in order to provide for his family, which resulted in the couple spending very little time together. Lisa also disclosed that they had not planned to have any more children after having the twins due to financial difficulties, but Lisa had become pregnant while taking the contraceptive pill. Lisa mentioned that on occasions, when Chris comes home in the evening, the twins would still be in their pyjamas and he was becoming increasingly frustrated with the situation; however, he was aware that Lisa was enduring a significant lack of sleep during the night due to the baby waking for feeds and understood she was exhausted in the day needing to meet the needs of two active three year old girls. Lisa stated she felt isolated and trapped by the situation but felt helpless and unable to improve how she was feeling due to the factors which were out of her control. The lack of social housing and the high cost of private rental properties meant that the family were unable to move. Lisa and Chris both had difficult relationships with their families and had limited social support. Lisas EPDS result was 18 which was of concern, however, research suggests that this screening process may not be leading to effective identification of PND, with studies finding that more than half the cases of PND are unrecognised by health visitors and GPs (Kulsar, 2011; Seeley et al, 1996). This family was chosen for discussion because a significant body of evidence has emerged including the recent reviews by Tickell (2011), Field (2010) and Marmot (2010) indicate that the initial first years of a childs life have a major impact on their overall health potential, and future happiness and wellbeing. The Millennium Cohort findings also suggest that during the early years of a childs life, maternal depression can have a significant effect on healthy attachment and bonding which can have profound effects later in life. Experiencing mental health problems is extremely difficult at any time, however, there are considerably greater implications when experienced during pregnancy and the postnatal period, possibly leading to serious consequences for the mother, her infant and family members (NICE, 2007). PND is defined by Robertson (2010) as a non-psychotic depressive condition that can often affect women in the first six months following childbirth. Conversely, Gibson et al. (2009) define PND as a mental and emotional disorder, which can often occur in women up to one year after child birth. Robertson (2010) identifies that the symptoms of PND do not differ from symptoms of mild to moderate depression experienced at other times except for the likelihood that the baby will often be the focus of the womens worries, thoughts and feelings, which develop into a deep longer term depression. The Confidential Enquiries into Maternal Death (2008) reported PND as the leading cause of maternal death in the UK. Th e enquiry reported 29 maternal deaths were as a result of suicide during pregnancy or within the first six months postnatal. WHO (2010) predicts that depression will be a leading primary cause of disability due to ill health by the year 2020. Mental health is a central public health issue that should be a priority and PND is a significant contributor to this public health issue, with a number of studies showing women are of increased risk of developing depression following childbirth than at any other time in their lives (Almond, 2009). There is a plethora of literature relating the prevalence of PND. The Department of Health (2011) and Royal College of Psychiatrists (2011) report that 1 in 10 mothers in the United Kingdom experience PND, which is the statistic frequently quoted. However, Almond (2009) argues that the true incidence is much higher, estimating that as many as 1 in 5 women suffer PND. Most episodes of PND resolve spontaneously within three to six months but evidence indicates 1 in 4 affected mothers continue to suffer for more than a year after childbirth (BMJ, 2010). Evidence from a recent study by Kulcsar (2011) reveals that 70,000 mothers in the UK suffer from PND each year and half of these mothers and their families suffer in silence each year. The sooner a mother with PND gets help, the less damaging it will be for her and her family, but early treatment depends on being able to recognise the condition. In the majority of cases the health visitor will be the first and only contact with individual or family experiencing difficulties in their mental health (CPHVA, 2009; Millar and Walsh, 2000). Therefore the health visitors role is pivotal in the detection of PNDthrough the screening procedures and during the subsequent referral process. It is essential therefore that the health visitor has the skills necessary for the detection of mental health problems and is able to deliver interventions that promote a clients mental health; early identification and support for families is a key intervention highlighted by WAG (2012). The health visitors role is to assess for depression using national recommendations (NICE, 2007). Identifying needs and recognising the appropriate level of need can be achieved through several approaches. Maslow (1954) introduced the concept of a hierarchy of needs. This hierarchy is often presented as a pyramid. The bottom level of the pyramid forms the most basic needs, while the more complex needs are located at the top of the pyramid. Needs at the bottom of the pyramid are basic physical requirements including the need for food, water, sleep, and warmth. Once the lower-level of need have been met, people can move on to the next level of needs, which are for safety and security. As people progress up the pyramid, needs become increasingly psychological and social. Soon, the need for love, friendship, and intimacy become important. Further up the pyramid, the need for personal esteem and feelings of accomplishment take priority. Like Carl Rogers, Maslow emphasized the importance of self-actualization, which is a process of growing and developing as a person in order to achieve individual potential. This assignment will now explore the framework utilised for the assessment of the familys needs. In cases where there is a perceived need The Welsh Assembly Government (2006) advocates the use of a holistic needs assessment to be carried out by the health visitor. The framework adapted and implemented by the local trust (Local Trust, 2008) is based on the Common Assessment Framework (CAF) (Department of Health, 2000) which offers an effective tool for early identification of additional needs. The CAF provides the health visitor with a consistent approach to the systematic collection and recoding of information that focuses on the childs needs and strengths, taking account the role of parents, carers and the impact of wider environmental factors on parental capabilities, in order to gain a holistic view of a familys situation. However, this would not be possible without effectively engaging the family within the process, this requires being open and honest with parents and appreciatin g that they may be fearful that they are being viewed as failing (Aldgate and Bradley, 1999). The structure of the assessment framework provides a basis for implementing support and appropriate intervention and a mechanism for the health visitor to make judgements on how best to support the family such as making appropriate referrals and sharing information within agencies in order to raise awareness within the multi-disciplinary team of the factors which are impacting on the familys health and well-being. The health visitor has a lead role in co-ordinating agencies in order to ensure that the family is not overwhelmed with contacts unnecessarily. The health visitors role also involves anticipating and reacting to needs as they present plus maintaining up-to-date, accurate documentation that includes a record of the involvement of other services.. In this way, provision of appropriate services are determined and made available as the need arises (DoH, 2004). Appleton and Cowely (2008) states the assessment of a familys health need is a vital element of health visiting practice and requires the health visitor to utilise many skill, knowledge and judgements to make a pivotal assessment in assessing need, safeguarding children and in determining levels of health intervention to be offered to children and their families. For the purpose of the case study the domains of the framework will be discussed individually concentrating on the childs developmental needs, parenting capacity and family and environmental factors. The Assessment Framework (DoH, 2000) assisted the student health visitor to contemplate the wider determinates of health such as the social, economic and environmental conditions which have an impact on health (Dahlgren and Wightehead, 1991). In relation to this case study the rationale for a follow up home visit was that Lisa had not been attending well-baby clinic with Amy as planned. The student health visitor had not previously met Lisa thus the home visit provided a valuable opportunity for her to form a relationship with Lisa in the security of her own home and facilitated the initial assessment (Streeting, 2010). Environmental and family factors The home visit was a valuable opportunity for the student health visitor to begin to make an assessment of the home environment in order to ascertain if it is a safe, appropriate environment for a child to live in. Lisa and Chriss flat appeared to be a relatively hygienic environment with necessities such as water/heating/sanitation facilities and cooking amenities; sleeping arrangements were suitable. The family were experiencing financial difficulties. Although Chris was employed the household income was low. Lisa was unemployed having previously worked in retail prior to having children, but due to lack of childcare the couple had made the decision that she would stay at home to look after the children. The effects of hardship can increase the likelihood of behavioural problems that can affect a childs ability to learn, which in turn affects educational and economic outcomes and these adverse behaviours can have long-term detrimental effects on the dynamics of low-income families (Duncan, Claussens, and Engel, 2004). Lisa and Chris live in an area with adequate amenities such as a local General Practitioners Surgery and pharmacy, with good transport system to the nearby towns. However, Lisa and the children had become isolated in their home due to Lisas current mental health issues and lack of support. Housing was a factor that was adding to the difficulties Lisa was experiencing, specifically the issue relating to leaving the flat and having no operational lift. Shelter (2012) recommend that housing and childrens services work together to collect information that identifies the impact of bad housing on outcomes. Completing a CAF will assists the health visitor to identify causes for concern and liaise with local the housing department where appropriate. In full consultation with Lisa, a letter was provide to the local housing authority highlighting concerns the family had. Joint Working Shelter believes that a stable home and immediate environment are fundamental to the wellbeing of children. Research by Shelter (2006) shows that outcomes are seriously undermined if there is failure to recognise problems and put a childs need for a stable and decent home at the heart of any practice. Lisa and Chris were awaiting a move to a house and at the point of writing this case study they we re still situated in the flat, however, the lift had been repaired but continued to be out of use on several occasions. As Lisas mental health was a cause for concern an assessment was undertaken by using the 10 question Edinburgh Postnatal Depression Scale (EPDS), (See Appendix 2). It is acknowledged as a valuable and effective way of identifying mothers experiencing depression (Cox et al, 1987). The EPDS is a tool that is simple but effective screening tool. Mothers who score above 13 are likely to be suffering from depressive symptoms. The EPDS score should not override clinical judgment. Lisa scored 18 out of 30 and it was agreed that she would make an appointment with her GP for further assessment, to confirm the diagnosis and discuss treatment options. Listening visits were then offered; this is a form of treatment provided by the health visiting service. A study by Turner et al (2010) indicated all 22 women who were interviewed after receiving this service reported the visits as beneficial, although many of them had also required additional intervention to manage their symptoms. For women with mild to moderate mental health problems, active listening provides opportunities for women to describe their experiences and tell their stories, and represent a useful way of establishing good rapport (Kulsar, 2011). A fundamental strategy in the promotion of maternal mental health is the use of effective skills to form a therapeutic relationship. Health visiting is recognised as being a relationship-based activity (Cowley, 1995 and Kendall, 1993). It is essential therefore that the health visitor has the skills necessary for the detection of mental health problems and is able to deliver interventions that promote a clients mental health through fostering good, open communication. Research by Saunders (2001) into assessing a familys needs and vulnerability has demonstrated that a client-centred, open approach promotes a trusting relationship that is empowering. Developmental needs The Assessment Framework DoH (2000) recognises that consideration should also be given to any social or environment disadvantages that can have an impact on the childs development. Amy was now seven weeks old and from holistic observation and assessment of her growth measurements she was at the normal stage of health and development for her age. Lisa was due to attend her 8 week postnatal visit with her GP the following week. The World Health Organisation (2008) reports that maternal mental health is inextricably linked with both physical and psychological development of children. Tackling the maternal mental health needs is likely to benefit these important outcomes. Field (2010) supports this, stating that maternal physical and mental health is a key driver of life chances throughout childhood which can have a major impact on inequalities in health and wellbeing as an adult. However, when King Henry VIII was ruling in Britain, Ireland was brought under British control. At the time, Britain was predominantly a Protestant country. Tension between the Catholic majority and Protestant minority began to arise in the two faiths. Throughout the years the British and Protestants began to tighten their grip and control in Ireland. In 1534 Henry VIII had the Ireland parliament declare himself as King of Ireland. The native Irish viewed the British as a major threat to their customs. There have been multiple uprisings and rebellions by the Irish people against the British.
